Rosie O’Donnell Explains Why She Decided to Reveal Her Secret Facelift

Rosie O’Donnell has opened up about getting a secret facelift, saying she decided to go public because she values honesty and authenticity. The comedian and former talk show host, 64, first revealed the cosmetic procedure in a recent Substack post, where she admitted feeling conflicted and “shameful” about the decision. She later explained more on the red carpet at the 2026 Tony Awards in New York City, where she said she wanted to tell the truth before a tabloid disclosed it first.
Speaking to E! News, O’Donnell said the current climate makes transparency especially important. She said people are “lying about everything all day” and that she wanted to share the “complicated emotions” she felt surrounding the surgery. O’Donnell said her decision was rooted in a desire for truth and love rather than secrecy, and she made it clear that she no longer wanted to hide something so personal.
O’Donnell also recalled that her 13-year-old autistic child strongly opposed the idea. According to the comedian, her child told her that if she went through with the procedure, she would no longer respect her. O’Donnell said that reaction gave her pause for several months, but she eventually went ahead with the facelift in January. When she returned home 10 days later, she said no one in her family noticed the change at first.
The comedian said one reason she chose to speak openly was to avoid being exposed by the media. She said she wanted to say, “This is what I did,” and to name the doctor in case others were interested. O’Donnell also joked that the surgery was extremely expensive, saying it cost more than any car she had ever bought, though she added that she could not “drive around in my face.”
O’Donnell said she does not plan to have more plastic surgery. She explained that the procedure addressed the specific concerns she had, including loose skin and two lines in her face. She also connected some of the skin laxity to her weight loss journey, saying she has used Mounjaro for the past three years, has diabetes, and has lost more than 50 pounds.
In her Substack post, O’Donnell said she was happy with the results. She described wanting to still look like herself, only “less haunted,” and said the surgery helped her stop “arguing with the mirror.” She also shared before-and-after photos on Instagram, showing the changes more openly. For O’Donnell, the reveal was as much about emotional honesty as appearance, and she presented the facelift as a personal choice she no longer wanted to keep private.
